Common Plan Set Errors We See

After reviewing civil plan sets across a range of project types, from commercial grading to residential subdivisions, patterns emerge. The same categories of errors appear repeatedly, across firms and project sizes. None of them are the result of careless work. Most are the result of the review process being harder than it looks.

What makes these errors worth studying is not how often they happen. It is what they cost when they get through. The categories below are ordered by consequence, starting with the ones that most often end up in a courtroom.

Drainage errors: the biggest source of claims

Drainage is where plan set errors do the most damage. A pipe invert that does not match between the plan and the profile sheet. A structure labeled with the wrong rim elevation. A storm line that reads one size on the utility plan and another on the detail. Each of these is a small inconsistency on paper. In the field, it becomes a pipe bust, a system that does not drain, or water going somewhere it was never supposed to go.

The failure mode that matters most: water on someone else's property. Flooding of adjacent parcels is one of the most reliable paths from a design error to a lawsuit, because the damaged party did not sign your contract and does not care about your liability cap. Drainage claims also tend to be discovered late, after paving and landscaping are complete, when the fix is expensive and the damage is documented.

Why these slip through: cross-sheet consistency requires holding two or more sheets in mind at the same time. In a large set, the plan and the profile might be 15 sheets apart. By the time a reviewer gets from one to the other, the specific number has faded.

ADA non-conformances: the errors that invite plaintiffs

Accessibility errors are unique among plan set issues: they can generate litigation even when nothing fails. Ramp running slopes that pass in isolation but exceed limits once cross slope is accounted for. Landings that are too small. Accessible routes that cross a grade break the design never resolved. Parking stalls and access aisles that do not meet dimensional requirements.

ADA compliance is checked by combining conditions that interact across sheets: running slope, cross slope, landing geometry, route continuity. A drawing can look right at a glance and still fail once the numbers are run together. And unlike most design issues, accessibility non-conformance has a built-in enforcement mechanism: private plaintiffs can and do bring these cases directly.

Grade busts: when the site doesn't work

This category is less about a standard and more about whether the grading actually performs. Spot elevations that do not tie to the adjacent callout. A swale that calculates flat, or reverse, when checked against the elevations along it. Pavement that slopes away from the inlet meant to drain it. Finished floor elevations that strand a doorway above or below the walk that serves it.

Individually these look like rounding noise. Collectively they are how a site ends up with ponding, drainage that never reaches the system, and rework during construction. They concentrate at the edges of the design, in the areas fit to constraints late in the process, which is exactly where reviewer attention runs out.

Utility conflicts and crossings

Utility disputes are consistently among the most common civil engineering claims. On the plan set, they start small: a storm line crossing a water main without the required vertical separation, a sewer lateral routed through a proposed light pole base, wet and dry utilities stacked in the same trench line without coordination, a crossing that was checked in one revision and invalidated by the next.

These errors are expensive because they surface during construction, in the ground, with crews standing by. The redesign happens under schedule pressure and the change order conversation is already adversarial.

Callouts referencing missing or wrong details

A plan sheet calls out "See Detail 7/C-8." Detail 7 on sheet C-8 does not exist. It was moved, renumbered, or removed during revisions. Or it exists but is a generic standard detail where a project-specific one was intended.

This category is almost entirely a revision artifact. Early in the project the reference was correct. Then a sheet was reorganized and the callout never got updated. The original author knew what they meant. A contractor in the field will not, and an RFI (or a guess) follows.

Missing notes and omissions

Errors get most of the attention, but omissions drive a large share of professional liability claims. A missing compaction note. A demolition limit that was never drawn. A required construction sequence that lives in the engineer's head instead of on the sheet. When something goes wrong in the field and the plan set is silent, the silence itself becomes the argument: the contractor did what the drawings said, and the drawings did not say enough.

Omissions are the hardest category for any reviewer to catch, human or otherwise, because you are looking for what is not there. Systematic review helps by checking each sheet against what a complete sheet of that type should contain.

Drainage study numbers that don't match the plan

The drainage report shows a peak flow through a given inlet. The inlet on the plan is sized for a lower flow. The plan was revised after the study was finalized, or the study was updated and the plan was not.

This is one of the harder errors to catch because it requires comparing the plan set to a separate document. It is also one of the more consequential: a drainage system sized for the wrong flow is a latent defect waiting for the right storm.

What the pattern tells us

Almost all of these errors share a root cause: something changed somewhere in the design, and the change did not propagate to every place it needed to go. That is not negligence. It is the natural consequence of designing a complex document iteratively, under schedule pressure, with multiple people making changes.

The solution is not a better checklist. It is a process that looks at the entire plan set, every sheet against every other sheet, every time, before submittal.
